Purpose: The purpose of the Board/Council Contract Performance Measures Data Reporting form is to collect Performance Measures Data.

Instructions: Enter the requested information and complete all.
Submit to DFPS – Until notified otherwise by DFPS, submit the completed form to DFPS in accordance with the following schedule:

Performance Period     Time Included                                  Report due
Period 1                           Sept, Oct, Nov, Dec, Jan, Feb       March 1-30

Period 2                          Mar, Apr, May, Jun, Jul, Aug        September 1-30
